2021 Scholarship Applications will be accepted beginning February 15, 2021 and must be submitted no later than April 1, 2021.
Awards will be determined by May 1, 2021 and applicants will be notified shortly thereafter.
Scholarship Application Request
The Application Request Form provides initial vital information to determine applicant eligibility and tracks the date and time of each request.
- All scholarship applicants must complete the Request Form, not a parent, friend or another person. A request submitted by anyone other than the applicant will be disallowed. Returning applicants may complete the application itself by selecting the option below.
- Following submission, the new applicants will receive a confirmation by email confirming receipt.
- Upon confirmation, the applicant will receive notification by email confirming eligibility. Applicants whose eligibility cannot be confirmed will also be notified, or be informed that additional information is required.
- Eligible applicants will be provided access to the Scholarship Application and Instructions.
